I was sent an online "Hiring Assessment". This is a 30 minute questionnaire comprised of several cross-checked "Strongly Disagree | Disagree | Neutral | Agree | Strongly Agree" questions asking about your work beliefs and past behaviors.

In the questionnaire, they strongly state that they want you to answer as you have behaved in the past, not how you would behave now having learned from past experiences. It's strictly a one-way trip through the questions: there's no way to back up and change any of your answers if you change your mind.
